The Clinic This Week

5-2-2012

Working this week were: Mary Blackmer NP, Rob Shook NP with RN's Pat Spragg and Barb Mrozinski. Two student nurses Amanda Gaylord and christina Eisenhauer. Julie Bonhom was our receptionist and also helped work on the filing system. We were able to treat 26 patients (8 new patients). 26 prescriptions were written, 4 samples were given, and one wrist splint. two x-rays were ordered, one was normal the other person had to come back for placement of an aircast. Patient's report they are very pleased with the appointments: "its good not too wait for 3 hours in a room full of sick grouchy people."

4-25-2012

Dr. A. Elmore and Rob Shook NP saw  25 patients. Nurse Bonnie Davis-laclow RN, Medical assistance April Porter, two nursing students assisted and providing patients care. 30 prescriptions were given, 10 medication samples were given. 3 patients were referred to the Medication Access portion of our Free Clinic to help them get the routine medications FREE.

4-11-12

On this day, NP Rob Shook and Mary Blackmer seen 27 patients (one was for elgibility).  Two patients needed x-rays, one return-to-work slip, and 5 refills on prescriptions.  33 prescriptions were written, 4 samples given out, 3 cart, 1 voucher, and 3 referrals.

3-27-12

Cindy Evans tended to 8 patients, 7 of these were follow-ups, 2 were suture removals, and she notified 2 patients of their lab results.

7 patients were seen by Dr. Barnes, 10 prescriptions were written, and 3 referrals

3-20-12

10 patients were seen by Dr. Verbrugge.  3 of these patients were sent for lab work, 4 were needing follow-up care, 1 required sutures, 2 prescriptions were given. 

3-14-12

A total of 26 patients were seen by Dr. Anderson and NP Rob Shook.  13 of these were new patients and the other 13 were return visits.  6 patients were sent for lab work, 2 patients requiring follow-up care, 36 prescriptions were given, 1 sample, 1 cart, 1 OTC, and 2 referrals.

Nurse Practioner Rob Shook and Mary Blackmer had a busy day with 26 patients.  2 labs were ordered, 2 x-rays, 32 prescriptions, 9 samples, 1 OTC, and 4 referrals were given. 

2-29-12

It was a busy day for Nurse Practioner Rob Shook.  Along with him Nurse LeBaron seen 31 patients, ordered 4 labs, 5 x-rays, dispensed 32 prescriptions, 4 samples, 2 over-the-counter medications, and 11 referrals.

2-28-12

Barnes and Cindy Evans seen 4 patients.  Of these 4 patients only 2 prescriptions were needed, but 3 samples were dispensed.

2-22-12

Dr. JoNel Scovel and Nurse Practitioner Rob Shook worked with RN Sharon Brazee and Student Nurses Andrew Maloney and Amy Ullom and Medical Assistant Externs Annmarie Clark and April Porter. Together these volunteers treated 45 patients. 45 prescriptions were written, 10 samples were given and referrals were given to Healthy Basics and to our own Medication Access Program.

2-15-2012

41 patients were seen by Nurse Practitions Rob Shook and Mary Blackmer. Nurses were Bonnie Schultz LPN and  Summer Dykhouse RN. Two Medical Assistant externs were April Porter and Annmarie Clark. Eligibility was completed by Mary Rosser, Rose Bulger and Deb Anderson. Reception was completed by Cindy Evans.
two labs were ordered. One Urinalysis and two strep tests were completed this evening. 37 prescriptions were written, 13 medication samples were given. One medication voucher was given. Two referrals were made to Tencon, One referral to Dr. Verbrugge, One referral to Cadillac Surgical Care, One referral to District Health Department #10. One referral to Cadillac Foot and Ankle.

2-8-2012

41 patients were seen by Rob Shook NP. Our Medical Assistant externs April Porter and Annmarie Clark worked with RNs Lucy McGuire and Cece Clark. Elegibility was completed by Bob Clark, Cathy Feister and Jim McGuire. Julie Peterson and Lynda Johnson were our receptionists. 11 labs were ordered, 3 ultrasounds and one x-ray. 55 prescriptions were ordered, 3 samples were given. One dental voucher was given and one medication voucher was given.

2-1-2012

39 patients were seen by NP Rob Shook and NP Pat Gable. MA Externs April Porter, Annmarie Clark, RN Darlene D'Arcangelo with LPN Bonnie Schultz assisted. 40 prescriptions were written and 6 free sample medications were given.

1-25-2012

Julie Peterson and Lynda Johnson were our receptionists, Kathy McCurdy and Mary Sorger completed eligibility information. NP Rob Shook and Dr. Moores saw 35 patients. RN Gayla Finstrom, LPN Bonnie Schultz, MA externs AnnMarie Clark and April Porter assessed, completed vital signs and discharge instruction. AnnMarie Clark and April Porter drew lab tests, saving our clients the trip to the hospital. Four x-rays were ordered (and completed at Mercy Hospital). One ear irrigation was completed, one injection for infection was given, One strep test was completed, One Urine dip analysis completed and One urine pregnancy test completed. 50 prescriptions were written, 7 sample medications were dispensed. One person was sent to the Emergency Room for symptoms out of our scope of practice.

1-18-2012

NP Rob Shook, Dr. Davin Much and NP Pat Gable treated 31 patients. Student Nurse Andrea Grix, Extern MA April Porter and Annmarie Clark along with RN Marie Robbins assisted with vitals, assessment and patient discharge. 59 prescriptions were written with one free sample dispensed.

1-11-2012

31 patients were seen by NP Rob Shook and NP Mary Blackmer.  MA Externs April Porter and Annmarie Clark, LPN Bonnie Schultz, Lucy McGuire, Student Nurses Dana Lopez and Brooke Smith assisted with vitals, assesment and discharges. 46 prescriptions were given.

1-4-2012

NP Rob Shook and Dr. Alicia Elmore saw 47 patients. MA externs AnnMarie Clark and April Porter, LPN Bonnie Schultz, and student nurse Damie Powell assessed, completed vital signs and discharge instruction. AnnMarie Clark and April Porter drew lab tests, saving our clients the trip to the hospital. 72  prescriptions were given along with 11 free samples.

12-21-2011

NPs Mary Blackmer and Rob Shook worked with Dr. Peltier to provide care to 38 patients. Nurses Mary Elenbaas RN, Gayla Finstrom RN worked with Students Damie Powell SN and April Porter SMA to help the clients understand the 53 prescriptions which were written and the 6 samples dispensed. One pregnance test was given, 4 labs and 2 xrays were ordered. 4 Dental vouchers were written and 2 medication vouchers were given to those unable to afford to fill their prescriptions. 

12-14-2011

Our providers were Rob Shook, with volunteer physician Dr. Wilson. The RN was Cece Clark, Student Medical Assistant April Porter and student nurses Sabrina Haight and Lori Platz also volunteered their time. With Jim McGuire and Bob Clark volunteering to do eligibility intake. Lynda Johnson was our receptionist. We treated 29 patients, wrote 36 prescriptions gave away 3 samples, One Dental voucher was given. There was a patient referral to Dr. Verbrugge and one to Cadillac Surgical.

12-13-11

Dr. Verbrugge and Pat Philo RN were the volunteers. They treated 8 patients, completing on excision with 6 sutures, Dr. wrote 2 prescriptions. One wound culture was taken. Two referrals were made, one to Cadillac Surgical and one to Great Lakes Family Care.

12-7-2011

18 days until christmas!
We treated 11 new patients and 13 friends returned. 4 lab draws were ordered valued at  268 dollars. One in-office pregnancy test was completed and a minor surgerical procedure called an Incision and Drainage was completed, estimated value of the two is 248 dollars. We provided two people with dental vouchers and one medication voucher. 23 prescriptions were written, 10 medication samples dispensed. Our volunteers were, Dr. Skinner, Dr. LeBaron and Pat Gable NP. Connie March RN, Sharon Brazee RN, Barb Mrozinski RN, Amanda Gaylord SN, and Fred West SN. Elegilibity was completed by Janine Skunda and Julie Bonham. Our receptionist was Lynda Johnson.

11-30-2011

Thirty Seven people were treated at the clinic for a dollar value of $5,500.00. 5 labs and 1 x-ray were order for a value of ($295.00). Our volunteers were Rob Shook NP and Pat Gable NP. Pat Spragg RN, Bonnie Schultz LPN, Cathy Feister and Kim Molitor.

11-23-2011

28 patients were seen, saving the $4,200.00 in provider visits. 3 received labs valued at approximately $215.00, 44 prescriptions were written, 2 sample medications were dispensed valued at $74.00. One sample was dispensed valuing $8.42 In office testing consisted of 2 A1c's (a  Diabetic test to reflect the blood levels over a long period) a urinalysis and a strep throat test a value of $93.00. Our workers were Rob Shook NP, Pat Gable NP, Pat Spragg RN, Bonnie Schultz LPN, and 2 student nurses. Gail Scarbrough and Julie Bonham completed eligibility. Cindy Evans Director was the receptionist.

11-16-2011

Five thousand seven hundred dollars in provider visits were given to 38 patients. Three received in office urinalysis, a $24 dollar value. 32 perscriptions were written, 4 samples were given. Two dental vouchers were given, and 3 prescription vouchers were given. Two clients were taken at Great Lakes Family Care by Rob Shook N.P. via the Mercy Grant Program. Our volunteers were Dr.  Moores, Dr. Scovel, Rob Shook, Bonnie Schultz LPN, Andrea Grix RN, Beth Oberhaus RN, Rose Bulger and Mary Rosser did eligibility, Lynda Johnson was receptionist.

This week at the clinic we served 35 patients (dollar value $ 4920.00). Our providers wrote 26 scripts. One person recieved a medication voucher for there prescription. Four samples of medication were given (dollar value $ 28.75).  Three Mercy Grants were given at no cost to the patient. And one x-ray was ordered for labs (value: $189) at no cost to the patient. One over-the-counter drug was given.

There was only one provider this week, Rob Shook FNP. Our nurses were Connie March RN, Bonnie Schultz LPN, Rachael Ruppert SN, and Lisa Snider SN. Elegibility was completed by Linda Weatheral and our director Cindy Evans. Our receptionist was Lynda Johnson. We treated 26 clients, a dollar value of $3,750.00. Three clients were sent for blood work and x-rays valueing $1,224.00. One surgical consult was ordered a value of $240.00 and will be provided next month at no cost to the client. Three in office tests were completed saving our patients $63.00, $48.00, and $28.00. 44 prescriptions were written, 4 samples were given an estimated value of $131.00. Two medication vouchers were given valueing approximately $89.00. One client was not seen because he was over income, he was referred to Great Lakes Family Care, Cadillac's own Federally Qualified Health Clinic, which bills clients on a sliding fee scale. One client was referred to the Health Department. Overall value of the evening was$ 5,573.00.

10/19/2011

Our providers this week were our Medical Director Dr. H. Kiomento M.D. and Rob Shook FNP. Nurses were Andrea Grix RN, Bonnie Schultz LPN and two student nurses. We served 35 patients (dollar value $4920.00). Our providers wrote 63 scripts. Three people were given medication vouchers for their prescriptions at a dollar value of approximately $132.00. Three sample medications were given (dollar value $23.00). Two surgical consults have been arranged at no cost to the patient (dollar value $480.00). Two patients were sent to Mercy Hospital for lab work at no cost to the patient (dollar value $312.60). One person was sent to Mercy Hospital for x-rays, at no cost to the patient (dollar value $273.00).

10/18/11

We served 11 patients (dollar value $1210.00), wrote 2 presecriptions, dispensed 1 medication (value $14.69), Two people were sent for labs (valued $216.60) one office procedure (valued at $45.00). The days total value was 42,940.60. Dr. Verbrugge and Pat Philo RN were the volunteers.

Week of 10/5/2011

We served 39 patients (dollar value $5850.00), wrote 64 presecriptions, dispensed 1 medication valued at $13.95), Two people were sent for lab work valueing $169.60. The evenings total care value was $6,033.55. Our providers were Dr. Mutch DO and R. Shook NP. Our nurses were Pat Philo, Jana DeVries and Darlene D'archangelo. Janine Skunda nad Julie Bonham volunteered to complete eligibility and our faithful receptionist was Linda Johnson.
